Eligibility, patient consent, clinical suitability, service requirements, and payer rules determine how a program can operate. Services are not automatically additive or separately billable. Your practice retains clinical and billing responsibility.

People, at the center

Care works through relationships.

Each person has a clear role. Everyone stays connected around the patient.

The patient

A life to fit care into.

The questions that arise at home deserve a place in the conversation. Continuous care starts by listening to what matters to the person living with the care plan.

Diagnosis and treatment remain with the medical practice.

A few things to know

Advanta supports continuous care between appointments. Medical-assistant care managers help patients stay connected to their care plan, nurses provide triage, and medical practices retain responsibility for diagnosis and treatment.

What happens in a panel review?

We discuss your specialty, approximate Medicare panel, existing care management activity, and workflow. That conversation helps identify a sensible starting point and the information needed to evaluate it.

Do you need patient records to begin?

No. The first request uses practice-level estimates. Please do not send patient names, medical records, or other protected health information through this website.
