Continuous care / Urology

A familiar connection for the questions patients hesitate to ask.

Some questions are easier to raise in a continuing conversation. Advanta helps urology practices keep patients connected between visits, with clear roles and a route back to the medical team.

One connected relationship

Between appointments

Listening is part of making support accessible.

Patients may need help understanding next steps, arranging follow-up, or finding the words for a concern. Continuous care gives those everyday conversations a dependable place.

Clear next steps

Support understanding of the practice’s existing care plan and follow-up instructions.

A continuing conversation

Care managers listen without rushing patients past the concern they want to raise.

Appropriate escalation

Nurses triage clinical concerns and connect with the practice through agreed pathways.

An everyday moment

A patient has a question they held back during a visit. The care manager makes room for it and connects them with nurse triage or the urology practice as appropriate.

Illustrative workflow, not an actual patient case or a clinical outcome claim.

A few things to know

Good questions.
Clear answers.

Does Advanta give patients medical advice?

Care managers support the practice’s plan and patient participation. Nurses provide triage. Diagnosis, prescribing, and treatment decisions remain with your medical providers.

What happens in a panel review?

We discuss your specialty, approximate Medicare panel, existing care management activity, and workflow. That conversation helps identify a sensible starting point and the information needed to evaluate it.

Do you need patient records to begin?

No. The first request uses practice-level estimates. Please do not send patient names, medical records, or other protected health information through this website.

A useful next step

Let’s talk about your urology practice.

Tell us about your patients, your team, and what you want to make possible between appointments.
