The days in between

The appointment is a beginning

A care plan has
a life to fit into.

At home, a patient works out the routines, finds new questions, and sometimes wonders whom to call.

A question comes up.

A detail that seemed clear in the exam room feels different at home.

A routine needs support.

Following a plan means finding a way to make it work in an ordinary day.

Someone stays connected.

A care manager listens. A nurse provides triage when a concern needs attention. Your practice stays responsible for care.

Advanta brings care managers, nurses, and practices together around the patient’s everyday life.

People, at the center

Care works through relationships.

Each person has a clear role. Everyone stays connected around the patient.

The patient

A life to fit care into.

The questions that arise at home deserve a place in the conversation. Continuous care starts by listening to what matters to the person living with the care plan.

Diagnosis and treatment remain with the medical practice.

Practice + care manager

A clear beginning.

The practice introduces the program. A care manager explains how support works, answers questions, and helps the patient understand enrollment.

Illustrative journey. These moments describe how roles can work together; they are not a patient testimonial or a promise of a particular schedule or outcome.

A few things to know

Good questions.
Clear answers.

What does Advanta do?

Advanta supports continuous care between appointments. Medical-assistant care managers help patients stay connected to their care plan, nurses provide triage, and medical practices retain responsibility for diagnosis and treatment.

Which services do you offer?

Our core services are Chronic Care Management (CCM), Behavioral Health Integration (BHI), Remote Patient Monitoring (RPM), Principal Care Management (PCM), and Advanced Primary Care Management (APCM). CCM is the foundation of our approach.

Which practices do you work with?

Our focus is specialty medical groups, including neurology, nephrology, rheumatology, urology, pulmonology, and multi-specialty groups. We also support primary care practices.

What happens in a panel review?

We discuss your specialty, approximate Medicare panel, existing care management activity, and workflow. That conversation helps identify a sensible starting point and the information needed to evaluate it.

Do you need patient records to begin?

No. The first request uses practice-level estimates. Please do not send patient names, medical records, or other protected health information through this website.

Are the published results a promise for our practice?

No. The figures describe Advanta’s internal observations or patients’ survey responses. Results vary. Each metric includes its evidence source so you can understand what it does and does not show.
