BHI services can generate meaningful revenue when combined with other connected care programs. A patient with diabetes, hypertension, and depression could be enrolled in CCM (99490 + 99439), RPM (99454 + 99457 + 99458), and BHI (99484) simultaneously, generating estimated combined monthly revenue exceeding $300 per patient.
