The following are approximate national average reimbursement rates for RPM services effective January 1, 2026.
Device Supply and Setup Codes:

99454 and 99445 are mutually exclusive. Bill one or the other based on the number of data transmission days. CMS finalized equal reimbursement for both codes, reflecting the rationale that practice expense costs remain the same regardless of transmission days.

99470 and 99457 are mutually exclusive. If total management time reaches 20 minutes, bill 99457 (not both 99470 and 99457). Additional time beyond 20 minutes is billed under 99458.
Maximum Monthly RPM Revenue Per Patient (2026):
Using the highest-engagement codes: 99454 ($47) + 99457 ($52) + 99458 x2 ($82) = approximately $181 per patient per month in ongoing RPM revenue. The one-time 99453 setup fee adds approximately $22 at enrollment.
Using the new lower-threshold codes: 99445 ($47) + 99470 ($26) = approximately $73 per patient per month for patients with shorter monitoring periods and briefer clinical interactions.
